Informed Consent to Participate in Research
Title: Exploring the Role of Parents in Deaf Cultural Identity Development

Purpose of Research: We want to understand two things:
· What parents say and do to teach their children what it means to be deaf or hard of hearing, and
· How your parent(s) influenced who you are now.
Procedures: Young adults who are deaf or hard of hearing are invited to take a survey for research. It is completely voluntary. You can always change your mind and drop out without consequences. Participants will take a survey online. It will take about 10-15 minutes. The survey will ask questions about your background and things your parent(s) may have said and done when you were a child. It will also ask you questions about how you see yourself today.
Benefits: You will receive a gift card worth $25 from a retailer or restaurant of your choosing.
You may not benefit personally from taking the survey.
The study could help parents know how to support their deaf or hard of hearing child.
The anonymous information collected from participants will be used to produce journal articles and conference presentations on the role of parents in Deaf cultural identity development.
Risks: The risk of participating is minimal. Some questions may be personal. You can skip them or quit the survey at any time.
Safeguards: The information you provide will be kept strictly confidential and your name and other identifying information will not be attached to your responses. Your data will be stored in a password protected data file. However, anytime you share any information online there are risks. Your data could be hacked or seen by someone who shouldn’t have access to it. We are using a secure system and collecting data anonymously to minimize this risk.
Your participation is completely voluntary, and you may withdraw from this study at any time without penalty, prejudice, or loss of benefits to which you would otherwise be entitled.

This project and this consent form have been reviewed by the Institutional Review Board at the University of Wisconsin-Green Bay, which ensures that research projects involving human participants follow federal regulations.
