INFORMED CONSENT FORM
Title: College Experience Group
Investigator(s): Kathryn Horner, Emily Kay, and Jake DeCremer
Purpose of Research: College experience in different students.
Procedures: Participants will be filling out a 10-to-15 minute survey that will measure their college experience and sports participation. To measure the college experience, we will be incorporating the basic needs satisfaction at college scale, or the BNSC-S, into our procedure. We will be incorporating a similar scale to measure college sports participation.
Benefits: Understanding the college experience and how it differs either positively or negatively when comparing athletes and non-athletes.
Risks: The risks associated with participation are minimal. Participants may experience normal anxiety associated with questions about oneself when it comes to the survey process and the questions they must answer.
Safeguards: The information you provide, and your identity, will be kept strictly confidential. Your data will be stored in a password protected data file. Your participation is completely voluntary, and you may withdraw from this study at any time without penalty, prejudice, or loss of benefits to which you would otherwise be entitled. Therefore, if you choose not to complete the final survey, you can still participate in the on-line study group. Questions concerning the research or receiving a report of the results of this study should be directed to Alison Jane Martingano at martinga@uwgb.edu.
I have been informed of the purpose, benefits, and risks of participating in this study. I have been given the opportunity to ask questions and have them answered to my satisfaction. I am at least 18 years of age, and I agree to participate in this study.
